ADOPTER’S AGREEMENT

• I agree that the animal is being adopted for myself and will not be sold or given to another party.

• I agree that the animal will not be allowed outdoors without proper supervision.

• I agree that I will not declaw (cat) nor perform any cosmetic surgery on the animal.

• I agree to bring the animal to PAWS when it reaches 6 months of age to be spayed at no costs (if not already spayed).

• I agree to care for the animal in a humane manner and be a responsible animal owner. This includes supplying adequate food, water, shelter, attention, and medical care.

• I agree that if, at any given point of time, I cannot keep the animal, I will return him/her to PAWS without requesting reimbursement.

• I understand and agree that PAWS makes no guarantees about the animal’s temperament and is not responsible for future damages or injuries caused by the animal.

• I give PAWS the permission to call my home at any reasonable time to ensure that the animal is being properly treated and cared for.

• I agree to keep PAWS updated about any change in my current home address and phone number.
